About Liberty ...

 

Liberty Independent Living Services was established in 2003.

 

Liberty Supported Living Services was established in 2009.

 

Liberty is a vendor of the North Bay Regional Center, which is funded through the Department of Developmental Services. Liberty currently serves over 50 Sonoma County residents with developmental disabilities.

 

Independent Living Services (ILS)

 

The Independent Living Service is designed to support consumers who are living on their own and want to improve their daily living skills. Our Instructors provide guidance and support with skill building in the areas of financial responsibilities, domestic duties, correspondence, communicating with outside agencies, and more.

Supported Living Services (SLS)

 

The Supported Living Service is designed to provide a more structured and thoroughly supportive plan. This ensures that the consumers served have a safety net of additional services in place, such as Personal Assistants, In Home Support Service workers, and an after-hours/weekend hotline for urgent (non-emergency) matters outside normal business hours. The program manager facilitates all services provided to ensure that the individual’s needs are met in the least restrictive living situation possible. Our staff assists consumers with developing and improving skills with money management, domestic duties, meal preparation, social activities, maintaining or improving their health and more.

 

*The program hours of operation are seven days a week and times vary depending on the needs of the consumer. After hours/weekend hotline is available 24/7. Consumers are able to contact their support staff by cell phone.
